In 1932, after 10 years of safe flight.
Like so many accidents since, avoidable.
Peter Brooks, in his book, reckoned that the Cierva machines were about 8x safer than GA at that time, and it took GA about another 40 years to reach the same level of safety as the early gyros. [Ironically, of course, the Bensen-type machines were going in the opposite direction by that time...]
