Well, based on their very cryptic list, with what we know so far, I'd guess:

"10. Intentionally left blank".

Hmmm ...I'd suspect #3 or#4 ...with the weather conditions ...Intentional/unintentional flight into IMC! leading to Spatial disorientation/loss of controlled flight!
